We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional’s point of view.
Home Health Care will reduce the length of your stay in a hospital, nursing home or rehab facility with these services. It can also postpone institutionalization and reduce the need for urgent care. In the convenience and comfort of your own home, we offer

Home Health is a way to monitor a health condition being treated at home along with strategies and training for overcoming or coping with a new disability, medical equipment, therapeutic diet or medications. We offer one-on-one care personalization to your needs. Services cost less than hospital or facility care.
Qualifying for Home Health Care
To qualify for home health care services through Medicare, you must have a medical need for the skilled service or services of a qualified healthcare professional.
You must also meet Medicare’s definition of “homebound”, which, generally, means that leaving the home requires taxing effort, causing you to, therefore, leave the home infrequently, if at all. A patient may be temporarily homebound due to an illness or injury, for example.
A Plus Home Health Care, Inc. must also receive an order from your physician to provide home health care services, which will lay out your specific plan of care. The A Plus Home Health Care, Inc. team will then coordinate with your physician directly.
If qualified, Medicare pays 100% of the fees for these services.
A Plus Home Health Care, Inc. provides home health care services in assisted living facilities as well as in private residences.
